（一）申请条件Application Conditions

补办《外国人工作许可证》的，申请人应当自证件遗失之日或发现遗失之日起在外国人来华工作管理服务系统上登载声明（申请发布遗失公告），并向许可决定机构申请补办。证件损毁的，申请补办时需携带原证。

If applying for a reissue of the "Foreigner's Work Permit," the applicant must, from the date of document loss or discovery of loss, publish a declaration on the Foreigner's Work Management Service System (apply for the publication of a lost notice) and apply for a reissue from the licensing authority. If the document is damaged, the original certificate must be presented when applying for a reissue.

（二）申请材料Application Materials

（1）外国人来华工作许可补办申请表Application Form for Reissuing Foreigner's Work Permit

在线填写申请信息后由系统自动生成，下载打印后由申请人签名，并加盖单位公章或经用人单位授权部门公章后上传至系统。注意申请人签名需与护照首页签名保持一致。

Fill out the application information online, and the system will automatically generate the form. After downloading and printing, the applicant must sign it and affix the official seal of the employer or the authorized department of the employer before uploading it to the system. Note that the applicant's signature must match the one on the first page of the passport.

所有纸质材料原件及中文翻译件均应以电子方式上传至办理系统，窗口受理时须提交所有上传至办理系统的材料的原件核验，材料电子版须与原件一致，除护照、工作许可证、居留许可需同时提交复印件外，其他材料无需提交纸质复印件。

All original paper documents and their Chinese translations must be uploaded electronically to the processing system. During window acceptance, all original documents uploaded to the processing system must be submitted for verification. The electronic version of the documents must match the originals. Except for passports, work permits, and residence permits, which require submission of photocopies along with the originals, no other documents need to be submitted in paper copy.
